If you own a business in the state of Ohio and wish to purchase parts from us tax exempt, please download the Ohio blanket exemption form, and fill it out.

When completed, please fax to 614-784-876, or email to bannon@midwest-bayless.com

Upon receipt of the form, we will validate that the vendor's license or Tax ID code is accurate, and update your account profile to register as tax exempt in the state of Ohio.

This will automatically remove any calculation of sales tax from your on-line or phone orders thereafter.
